Lupus vulgaris is commonest form of cutaneous tuberculosis. It is a chronic and progressive disease with great variability in clinical presentation. It’s different variants are plaque form, ulcerative and mutilating form, tumour like and vegetative form. There are many reports on different form of Lupus vulgaris but there are only a few reports on large sized Lupus vulgaris. Here we report a case of 64 years male with giant Lupus vulgaris who presented with large plaque over right lower limb with contracture over knee. Histopathology showed numerous granulomas with multinucleated giant cells. The Mantoux test showed strong positivity. Case is being reported because of its extreme chronicity of 40 years duration and unusually large size.
